Long-Term Care Insurance: Planning For The Care You May Need Someday

September 30, 2026•2 min read

Key Points

  • Long term care insurance may help cover eligible care beyond nursing homes, including in home care, assisted living, and other qualifying services.

  • Medicare and health insurance may not cover extended custodial care, which can leave families responsible for significant care costs.

  • Long term care planning can take different forms, and the right approach depends on your health, finances, family needs, preferred care setting, and policy design.

More Than Nursing Home Care

Long-term care is often associated with nursing homes, but the need can be much broader. Depending on the policy, long-term care insurance may help pay for eligible services such as in-home care, assisted living, adult day care, respite care, or nursing facility care. The goal is generally to help with ongoing assistance when an illness, disability, or cognitive impairment makes everyday activities more difficult.

The Coverage Gap

Health insurance and Medicare should not automatically be assumed to cover extended custodial care. Medicare covers certain qualifying skilled care, while Medicaid has separate eligibility requirements that may include financial criteria. This can leave families with an important question: if extended care becomes necessary, how will it be paid for?

How Benefits Work

Long-term care policies can differ substantially. Coverage may depend on benefit amounts, benefit periods, elimination periods, covered settings, eligibility requirements, inflation protection, and other policy provisions. An elimination period, for example, may require you to pay eligible care expenses for a period of time before benefits begin. Inflation protection can also be important because the cost of care years from now may be very different from the cost today.

More Than One Way to Plan

Traditional long-term care insurance is only one approach. Some life insurance policies and annuity contracts may also offer long-term care benefits or riders. Each approach can have different premiums, benefits, guarantees, limitations, underwriting requirements, and consequences if you never need care. Tax-qualified long-term care policies may also receive certain federal tax treatment, depending on individual circumstances.

The Question Is Personal

The important question may not simply be, “Do I need long-term care insurance?” A better question may be, “If I needed help for several months or years, what would I want that care to look like, and how would I want to pay for it?” Your age, health, financial resources, family circumstances, preferred care setting, and existing insurance may all affect that conversation.

Curious about what long-term care planning could look like for you or your family? A conversation can help you understand the available options, what they may and may not cover, and which questions are worth asking before deciding.
